DESCRIPTION
Alprazolam is a triazolobenzodiazepine tranquilizer approved for the treatment of anxiety, general anxiety disorder, and panic disorder. It affects the electrical activity of key neurons in the brain.

More recently, it has been used off-label in combination with other medications in the treatment of Irritable Bowel Syndrome (IBS). A significant proportion of patients with generalized anxiety disorder have IBS.

POTENTIAL SIDE EFFECTS
Lightheadedness, changes in libido, menstrual disorders, dry mouth, appetite increase or decrease, weight gain/loss, low blood pressure, confusion, derealization, dizziness, disinhibition, dermatitis, rash, difficulty urinating, salivation increased, upset stomach, sexual dysfunction, rigidity, tremor, muscle cramps, nasal congestion, muscle aches, shortness of breath, drowsiness, fatigue, difficulty walking, sweating.

CAUTIONS
  • Do not suddenly stop therapy; if you stop using this drug, your use should be tapered gradually. Withdrawal symptoms, including seizures, have been reported 18 hours to three days after abrupt discontinuation or after large decreases in doses.
  • Paradoxical reactions have been reported with use (excitability, anger, hostility).
  • Elderly people should use this drug with caution (may require smaller doses). Cautions should also be used by debilitated patients, those with reduced liver or kidney function, obese patients, patients with history of dependence or depression.
  • May cause anterograde amnesia (memory loss of recent actions, events).
  • Use with caution if you are receiving concurrent CYP3A4 inhibitors (see drug interactions).
  • May cause sedation and impair physical or mental capabilities. Concurrent use with other sedatives may increase the risk of severe sedation.

Narcotic analgesics, barbiturates, phenothiazines, antihistamines, MAO inhibitors, sedative-hypnotics, cyclic antidepressants, imipramine and desipramine.

Grapefruit juice, alcohol.

Valerian, St. John's wort, kava kava, gotu kola, gingko biloba

XANAX FOR IRRITABLE BOWEL SYNDROME
The Journal of Clinical of Psychiatry reported on a study of alprazolam in the treatment of irritable bowel syndrome. The goal was to investigate whether treatment of generalized anxiety disorder would influence the course of IBS symptom severity. Investigators gave Xanax to 32 patients for six weeks, and 94% of patients had full or partial alleviation of anxiety. Eighty-nine percent experienced a concomitant reduction in the severity of their irritable bowel syndrome. For the majority, benefits continued to be felt as the drug was tapered over four weeks and for four weeks after its discontinuation. The researchers concluded that alprazolam treatment was safe, effective, and beneficial for IBS, with only limited post-treatment rebound of symptoms.

ALPRAZOLAM FOR MAJOR DEPRESSIVE DISORDER
The effectiveness of alprazolam in treating major depressive disorder has been studied in comparison with drugs of the tricyclic antidepressant class, since the latter are known to be effective. Xanax was compared with desipramine, a tricyclic antidepressant, in three studies with varying results. In one study, 52 depressed patients took alprazolam or desipramine for six weeks. The two drugs were equally effective in treating their depression, and neither drug gave troublesome side effects.

In the second study, patients were given Xanax, desipramine, or a combination of alprazolam and desipramine for six weeks. All three treatments gave similar degrees of improvement in symptoms of depression. However, there was a difference in the time course of antidepressant effects among the treatment groups. Patients taking Xanax experienced relief of their depressive symptoms earlier than those not taking alprazolam, with significant improvement often occurring within the first week of treatment. In the third study, 54 patients were given either alprazolam or desipramine for six weeks, but the results were different from those in the other two studies. Neither alprazolam nor desipramine was particularly effective in treating depression. Researchers believe the variation in results could be due to differences in the severity of depression among the patients.
