DESCRIPTION
Clonazepam is a benzodiazepine that has anxiolytic (suppresses anxiety), sedative, and anticonvulsant properties and belongs to a class of drugs called central nervous system (CNS) depressants. It is used in the treatment of panic and seizure disorders. Side effects include drowsiness, clumsiness, ataxia, behavior problems, rash, back pain, blood disorders, increased salivation, and withdrawal symptoms. It also tends to become addictive. Clonazepam adds to the effect of alcohol and other CNS depressants (sleep aids, muscle relaxants, antihistamines, and pain drugs) and may lose efficacy with cigarette smoking.

Clonazepam has shown benefit in the treatment of tremor, pain, and ataxia (uncoordinated movement), eye movement disorders, muscle spasms, stiffness, postural tremor, and walking difficulties associated with multiple sclerosis. Although limited scientific data are available on this drug and MS, as a sedative/anticonvulsant it has helped control symptoms by depressing CNS activity. It may be difficult to distinguish between common symptoms of multiple sclerosis and certain side effects of clonazepam; therefore patients should communicate any reactions they notice. Klonopin has also been used in the treatment of Parkinson's disease.

POTENTIAL SIDE EFFECTS
Drowsiness, sedation, lethargy, apathy, amnesia, memory impairment, confusion, crying, headache, slurred speech, constipation, dry mouth, changes in libido, urinary retention, vision changes, behavior problems, excitement, psychosis, increased salivation.

CAUTIONS
  • Notify your doctor if you have an allergy to this drug or other benzodiazepines, if you have liver or kidney problems, mental depression, a history of drug dependence, narrow angle glaucoma, lung disorders, increased salivation, a poor or problematic gag reflex, or are pregnant.
  • Use with caution in the elderly or debilitated patients.

  • Do not abruptly stop this medication, requires tapering of dose over gradual period.
  • Prolonged use for anxiety may result in dependence.
DRUG INTERACTIONS
Valproate, other drugs which cause sedation or cause central nervous depression.

FOOD INTERACTIONS
May take with food or water if stomach upset occurs.

St. John's wort, valerian, kava kava, gotu kola

FDA Pregnancy Risk Category D. Excreted in breast milk. Refrain from breast-feeding while taking this drug.

CLONAZEPAM FOR SOCIAL PHOBIA
In a study with 23 patients, clonazepam was compared to no treatment for eight weeks. Researchers observed that Klonopin had a significant effect on the patients. Although 70% of the patients experienced sedation when they started taking the drug, this effect wore off with time or when the dosage was reduced. The effectiveness of this drug was also studied in a larger group of 75 patients who were given Klonopin or placebo for up to 10 weeks. Clonazepam treatment was successful in 78% of the patients, while only 20% of those on placebo had positive results. Significant differences in the two groups were evident in the first week of treatment.

Clonazepam was also studied as an add-on treatment in patients who were taking another drug, paroxetine. Patients were divided into two groups and given paroxetine plus clonazepam or paroxetine plus placebo. The addition of clonazepam to paroxetine made little or no difference. The percentage of patients who experienced relief from their symptoms was approximately the same in both treatment groups. In another study, 75 patients received either clonazepam or paroxetine for a brief period and then were re-evaluated after two years. Researchers noted that even after two years, the patients who had taken clonazepam experienced relief from their social phobia.
