DESCRIPTION
Desogestrel ethinyl estradiol is used most commonly to prevent pregnancy or regulate the menstrual cycle. It may also be used for treating acne.

CONTRAINDICATIONS
Thrombophlebitis, thromboembolic disorders, history of deep vein thrombophlebitis, cerebrovascular disease, MI, CAD, known or suspected breast carcinoma or estrogen-dependent neoplasia, carcinoma of endometrium; hepatic adenomas, carcinomas; past or present angina pectoris, undiagnosed abnormal vaginal bleeding, cholestatic jaundice.

PRECAUTIONS
Hypertension, gallbladder disease, CHF, diabetes mellitus, depression, migraine headache, seizure disorders, hepatic disease, family history of breast or endometrial cancer, history of thromboembolic disorders, uterine fibroids, hypertriglyceridemia, hypercalcemia, cigarette smoking (especially >35 yr).

POTENTIAL SIDE EFFECTS
Depression, migraine headache, emotional lability. Arterial thromboembolism, pulmonary embolism, CVA, MI, hypertension, venous thrombosis edema. Contact lens intolerance, retinal thrombosis. Nausea and vomiting, gallbladder disease, bloating, benign hepatic tumors mesenteric thrombosis. Breakthrough bleeding (80% resolve in 3 mo), spotting, amenorrhea, change in cervical secretions, breast enlargement, breast tenderness. Hyperglycemia, hypertriglyceridemia, hypercalcemia, vitamin B-6 deficiency. Melasma.

CAUTIONS
  • Use drug with caution in patients with hypertension; convulsive disorders; migraines; asthma; cardiac, hepatic, or renal dysfunction.
  • Oral contraceptives may cause an increase in size of preexisting uterine leiomyomata (fibroids).
  • Some products may contain tartrazine, which may cause allergic-type reaction in susceptible individuals.
  • Acute intermittent porphyria. May be precipitated by estrogen therapy in susceptible individuals.
  • Use drug with caution in patients with history of depression.
DRUG INTERACTIONS
Warfarin, barbiturates, carbamazepine, griseofulvin, phenytoin, penicillins, rifampin; rifabutin, ritonavir, tetracyclines, corticosteroids, cyclosporine.

FOOD INTERACTIONS
Grapefruit juice (increased ethinyl estradiol concentration).

HERBAL INTERACTIONS
St. John's wort, dong quai, black cohosh, saw palmetto, red clover, ginseng

PREGNANCY AND BREAST-FEEDING CAUTIONS
FDA Pregnancy Risk Category X. Do not use in pregnancy. Excreted in breast milk and not recommended during breast-feeding.

SPECIAL INFORMATION
  • Smoking: Increased risk of oral contraceptive-induced adverse cardiovascular events.
  • Give at same time each day. Efficacy depends on strict adherence to dosage schedule.
