DESCRIPTION
Erythromycin is an antibiotic approved to treat a wide range of infections caused by bacteria, such as streptococcal infections of the skin, ears, and sinuses, pneumococcal pneumonia, chlamydia, bronchitis; diphtheria; Legionnaires' disease; whooping cough; pneumonia; rheumatic fever; certain venereal diseases, and other infections. It is also approved for use before some surgery or dental work to prevent infection in certain people at risk for complications.

Acne, chancroid, cholera, diarrhea due to Campylobacter, gingivitis, granuloma inguinale, impetigo, Lyme disease, lymphogranuloma venereum, periodontitis.

POTENTIAL SIDE EFFECTS
Systemic: Irregular heart rate (ventricular arrhythmias, QT prolongation), headache, fever, seizures, rash, abdominal pain, cramping, nausea, oral yeast infection, vomiting, jaundice, inflammation at the injection site, weakness, cough. Topical: skin reactions including redness, peeling itching, dryness.

CAUTIONS
  • Do not use if you have pre-existing liver disease and use with caution if you have reduced liver function.
  • Do not use concomitantly with ergot derivatives, pimozide, astemizole, or cisapride.
  • May aggravate weakness symptoms if you have myasthenia gravis.
DRUG INTERACTIONS
Cisapride, gatifloxacin, moxifloxacin, pimozide, sparfloxacin, thioridazine, alfentanil, benzodiazepines (such as diazepam, flurazepam, etc.), warfarin, buspirone, tegretol, cyclosporine, digoxin, ergotamine and ergotamine-like drugs, disopyramide, felodipine, fluoroquinolones (such as ciprofloxacin, norfloxacin, etc.), statins (such as lovastatin, fluvastatin, etc.), methylprednisolone, rifampin, theophylline.

FOOD INTERACTIONS
Avoid alcohol. The absorption of some erythromycin formulations are affected by food. Er. stearate should be taken at least two hours before or after a meal. Er. estolate, er. ethylsuccinate and erythromycin base may be taken without regard to meals.

HERBAL INTERACTIONS
St. John’s wort.

PREGNANCY AND BREAST-FEEDING CAUTIONS
FDA Pregnancy Risk Category B. Excreted in breast milk. This drug is considered compatible with breast-feeding by the American Academy of Pediatrics.

SPECIAL INFORMATION
Erythromycin is also used off-label to treat a group of bacteria called Campylobacter, one of the most common bacterial causes of diarrhea disease in the United States and worldwide. Although ciprofloxacin is often prescribed for those diagnosed with Campylobacter, it has become resistant to it in some areas, and Ilosone has become the antibiotic of choice, according to an article in eMedicine from the State University of New York at Stony Brook. Researchers believe that Ilosone resistance remains low, and it can be used in children or women who are pregnant, in contrast to problems presented by ciprofloxacin in those patients.

A Norwegian study examined in the laboratory the susceptibility to erythromycin and ciprofloxacin of 296 Campylobacter jejuni strains isolated during the 1998-1999 period. They found that nearly one in four of the isolates likely from Norway was resistant to ciprofloxacin; all resistant isolates were acquired outside Norway. In contrast, researchers state that resistance to Ilosone was very rare, indicating that it should still be the drug of choice for campylobacteriosis.
