DESCRIPTION
Fluticasone works directly in the lungs to make breathing easier by reducing the swelling and inflammation of the airways. This medication must be used regularly to prevent the wheezing and shortness of breath caused by asthma, bronchitis, or some types of emphysema.

POTENTIAL SIDE EFFECTS
Oral inhalation: Headache, fever, nausea/ vomiting, viral gastric infection, diarrhea, abdominal discomfort/pain, muscle injury, musculoskeletal pain, back problems, upper respiratory tract infection, throat irritation, nasal congestion, sore throat, viral infection.

Nasal inhalation: Headache, sore throat, nausea/vomiting, epistaxis, asthma symptoms, cough, aches and pains, flu-like symptoms.

CAUTIONS
  • Inhaled fluticasone should not be used for primary treatment of acute asthma attacks.
  • May cause hypercorticism or suppression of hypothalamic-pituitary-adrenal (HPA) axis, particularly in younger children or in patients receiving high doses for prolonged periods. HPA axis suppression may lead to adrenal crisis.
  • Particular care is required if you are being transferred from oral or injection corticosteroids to inhaled products due to possible adrenal insufficiency or withdrawal from steroids, including an increase in allergic symptoms.
  • Concurrent use of ritonavir (and potentially other strong inhibitors of specific liver enzymes) may increase fluticasone levels and effects on HPA suppression.
  • Controlled clinical studies have shown orally-inhaled and intranasal corticosteroids may cause a reduction in growth velocity in children.
  • May suppress the immune system, so you may be more susceptible to infection. Use with caution, if you use this drug at all, if you have systemic infections, active or quiescent tuberculosis infection or ocular herpes simplex. Avoid exposure to chickenpox and measles.
  • Flovent Rotadisk contains lactose; very rare allergic reactions have been reported in patients with severe milk protein allergy.

Fluticasone is not indicated for the relief of acute bronchospasm. The inhaled products have a black box warning which describe the need for special care in patients who are transferred from systemic (oral or injectable) steroids to inhaled products that are less absorbed. A sudden decrease in dose due to lesser absorption may result in fatal suppression of the hypothalamus-pituitary-adrenal function and cause an adrenal crisis. This particularly applies to patients that are maintained on at least 20 mg daily (or its equivalent).

Inhaled fluticasone propionate is widely used to reduce lung inflammation in chronic obstructive pulmonary disease, but the potential effects of fluticasone propionate on airway cells from patients with Cystic Fibrosis (CF) are unknown. In CF, an inflammatory lung response occurs through a defective protein that makes the body susceptible to the bacteria Pseudomonas aeruginosa, which causes the airway to become filled with a thick mucus. The presence of these bacteria in the body leads to a chronic inflammation that further damages the lung cell surface.

Corticosteroids such as fluticasone have long-term benefits in suppressing the potentially damaging inflammatory response within the airways of patients with CF. Fluticasone proprionate is especially helpful in children with cystic fibrosis who are wheezy and have associated asthma. A study conducted in France found that this drug exerts an antiinflammatory effect by blocking a signal leading to a reduced level in lung cells.
