DESCRIPTION
The anticancer therapy letrozole belongs to a class of drugs known as aromastase inhibitors, which block aromastsase (an enzyme) that is a major source of estrogen (a female hormone) in the body. Use of Femara prevents almost all estrogen production. Typically, letrozole is used to treat certain forms of breast cancer. Side effects associated with this drug include back, bone, joint, and muscle pain.

This drug may be helpful in men who are infertile because of abnormal hormone levels. Clinical studies specifically evaluating the use of letrozole in infertile males are limited. In a study in The Netherlands, the use of Femara normalized testosterone in severely obese men with hypogonadism (abnormally decreased functioning of the testicles and/or retardation of growth or sexual development). In this study, 10 severely obese men were given letrozole for six weeks, after which estradiol levels decreased and testosterone levels increased. Researchers concluded that short-term use of Femara, as dosed in this study, normalized testosterone levels.

TREATMENT OF INFERTILITY IN WOMEN
The effects of letrozole and clomiphene citrate were studied in women undergoing superovulation (stimulating the ovaries to develop multiple eggs) and intrauterine insemination (IUI) have been compared. A total of 238 cycles of superovulation and IUI in women with infertility. Women received either letrozole daily or clomiphene citrate daily.

There was no significant difference between the total number of developing follicles in the letrozole and in the clomiphene citrate groups. No difference was found in the endometrial thickness between the two groups. The pregnancy rate per cycle was 11.5% in the letrozole group and 8.9% in the clomiphene citrate group. Four of the 11 pregnancies in the clomiphene citrate group resulted in a miscarriage (36.6%). The investigators concluded that superovulation and IUI with letrozole and clomiphene citrate are associated with similar pregnancy rates, but the miscarriage rate is higher with clomiphene citrate. The ideal dose of letrozole remains unknown and further study is needed.
