DESCRIPTION
Levonorgestrel and ethinyl estradiol belong to the class of medication known as oral contraceptives (OCs) or birth control pills, usually containing two types of hormones, estrogens and progestins. When these hormones are taken together they interfere with the normal hormonal fluctuations during the menstrual cycle and maintain them at a more constant level. This medication can also help reduce excessive production of androgens, body hair growth, acne and painful menstruation in polycystic ovary syndrome (PCOS).
Because the abnormal tissue in endometriosis is sensitive to menstrual cycle hormones, oral contraceptives are widely used to treat it and are often the first line of therapy for endometriosis.

POTENTIAL SIDE EFFECTS
Intermenstrual bleeding and irregularity, abdominal pain, diarrhea, nausea, vomiting, depression, dizziness, emotional lability, headache, migraine, nervousness, change in vision or ability to wear contact lenses, fluid retention, darkening of skin in areas, vaginal infections, hair loss. Increased risk of cardiac events, glucose intolerance, stroke, liver problems.
CAUTIONS
- Oral contraceptives do not protect against sexually transmitted diseases or HIV infection.
- Cardiovascular events increase if you are over 35 years of age and heavy smoker (at least 15 cigarettes daily), or have a history of cardiovascular disease or thromboembolic (blood clots) events.

ALESSE AND POLYCYSTIC OVARY SYNDROME
Combination oral contraceptives are the most often used treatment for PCOS as they suppress androgen production, thus reducing skin androgenic symptoms and improving menstrual dysfunction.
On the other hand, much still remains unknown about their metabolic effects. COCs could decrease insulin sensitivity and affect glucose tolerance, although the negative influence on insulin sensitivity is dependent on other factors (especially obesity) that may not be an issue if you are not obese.
This impairment of glucose tolerance may be reversible, as the incidence of diabetes is not increased in past COC users. A study looked at whether a multi-drug that combines oral contraceptives and metformin is beneficial compared to COC alone.

Two dropped out. Investigators concluded that adding metformin slightly modified the effect of COC, causing a more significant decrease in testosterone.
Researchers concluded that the available data does not offer enough evidence to advocate the standard use of combined treatment in PCOS, but the combination might be beneficial for specific subgroups.
