DESCRIPTION
Medroxyprogesterone acetate (MPA) oral is a progestin hormone medication. Oral progestins are the oldest drugs used for endometriosis. Progestins can prevent ovulation and reduce the risk for endometriosis in two ways. They reduce luteinizing hormone (LH), one of the reproductive hormones important in ovulation and they also change the lining of the uterus and eventually cause the lining to waste away. Some experts recommend progestins as the first choice for women with endometriosis who do not want to become pregnant. Progestins should not be taken during the luteal phase of the menstrual cycle (the premenstrual phase, which is 14 days before a period.)

MPA is typically administered orally for three months. Pain relief with progestin therapy has been reported as excellent. Clinical trials have shown a pain relief rate of up to 90% and improvement of pelvic density and tenderness in 80% of patients. Progestins have not been shown to improve fertility. Studies of women with early-stage endometriosis who were treated with MPA or expectant management had similar pregnancy rates over an 18-month period of time. Adverse effects include abnormal vaginal bleeding in about 20%, nausea, breast tenderness, fluid retention and depression. If effective, researchers have found that this medication can be used safely for long periods of time.

ORIGINAL USES (ON-LABEL)
Endometrial cancer, kidney cancer, secondary amenorrhea or abnormal uterine bleeding due to hormonal imbalance, reduction of endometrial hyperplasia in nonhysterectomized postmenopausal women receiving conjugated estrogens.

NEWLY DISCOVERED USES (OFF-LABEL)
Chronic pelvic pain, endometriosis, hirsutism, McCune-Albright syndrome, menopausal symptoms, pelvic congestion, polycystic ovary, precocious puberty.

POTENTIAL SIDE EFFECTS
Dizziness, headache, nervousness, decreased libido, menstrual irregularities (bleeding, amenorrhea, or both), abdominal pain/discomfort, weight changes, weakness, leg cramps, joint aches.

CAUTIONS
  • This drug should be discontinued at least 4–6 weeks prior to surgeries associated with an increased risk of blood clots or during periods of prolonged immobilization.

DRUG INTERACTIONS
Aminoglutethimide, antibiotics (rifampicin and rifabutin), antiepileptic drugs (carbamazepine, phenobarbitone (phenobarbital) phenytoin, primidone, and topiramate), griseofulvin, nevirapine, tretinoin reduce the contraceptive effects of progestogens.

FOOD INTERACTIONS
Absorption increased if the oral tablet is taken with food.
