DESCRIPTION
Metoclopramide has a number of uses: it is a dopamine-receptor antagonist, an anti-emetic (drug that stops or prevents nausea and vomiting), and a stimulant of upper gastrointestinal (GI) activity. It is used in a wide variety of GI disorders, particularly gastroesophageal reflux disease (GERD), and to prevent chemotherapy-induced nausea and vomiting.

Diabetic gastric stasis, gastroesophageal reflux disease (GERD).

NEWLY DISCOVERED USES (OFF-LABEL)
Hyperemesis gravidarum, migraine treatment, nephrogenic diabetes insipidus.

POTENTIAL SIDE EFFECTS
Restlessness, drowsiness, extrapyramidal symptoms (neurological side effects), diarrhea, weakness, rash, breast tenderness, nausea.

CAUTIONS
  • Use in patients who have Parkinson’s disease or history of mental illness has been associated with extrapyramidal symptoms and depression. Extrapyramidal symptoms are characterized by involuntary movements of the limbs, facial grimacing, rhythmic protrusion of the tongue, etc.
  • Extrapyramidal reactions typically occur within the first 24–48 hours of use and risk is higher in children and adults more than 30 years of age and at higher doses.
  • Neuroleptic malignant syndrome (such as severe high blood pressure, tremors, etc.) has been reported (rare) with metoclopramide use.
  • May cause transient elevations in serum aldosterone. Use with caution if you are at risk of fluid overload.
  • Use caution if you have seizure history.
  • Withdrawal symptoms may result (rare) upon abrupt discontinuation.
  • Dose must be adjusted if you have kidney impairment.

FDA Pregnancy Risk Category B. Excreted into breast milk. The American Academy of Pediatrics classifies metoclopramide as a drug for which the effect on a nursing infant is unknown but may be of concern.

SPECIAL INFORMATION
Immediately notify your doctor if involuntary movements of the eyes, face, or limbs occur. May produce drowsiness; use caution when performing tasks that require alertness. Take medication 30 minutes before meals.

Researchers unexpectedly found that the addition of magnesium to Reglan may reduce the effectiveness of metoclopramide in relieving migraine.
