DESCRIPTION
Metronidazole tablets are an antibiotic used to kill or prevent the growth of bacteria or other microorganisms and is approved for the treatment of inflammatory lesions and rosacea, vaginosis and vaginitis. It is used off-label to treat hepatic encephalopathy by reducing the excessive amount of ammonia-producing bacteria in the body.

ORIGINAL USES (ON-LABEL)
Treatment of susceptible anaerobic bacterial and protozoal infections, topical treatment of inflammatory lesions and rosacea, bacterial vaginosis and vaginitis due to susceptible organisms (vaginal preparations).

NEWLY DISCOVERED USES (OFF-LABEL)
Crohn's disease, diverticulitis, gingivitis, hepatic encephalopathy, periodontitis, tetanus.

POTENTIAL SIDE EFFECTS
Uncoordinated gait, confusion, dizziness, hives, disulfiram-like reaction (flushing, nausea, vomiting), nausea, vomiting, constipation, diarrhea, metallic taste, darkened urine, blood disorders (low white blood cell count), weakness, flu-like syndrome.

Vaginal preparation: stomach cramps, abdominal cramps, vaginitis, vaginal itching or swelling.

Topical preparation: skin redness, itching, rash, dryness, worsening of rosacea.

CAUTIONS
  • Notify your doctor if you have kidney or liver impairment, a history of seizures, blood disorders, or congestive heart failure.
  • Therapy should be discontinued if seizures or numbness develops. Dose may need adjustment in elderly.
  • May need to monitor blood for potential blood disorders (oral and intravenous formulations).
  • Carcinogenic potential has been demonstrated in animal studies with chronic oral administration in rats and mice.
  • Intravaginal preparations: Avoid use with eyes. Gel may contain ingredients that cause burning or stinging.
  • Topical preparation: Avoid contact with eyes. For external use only.
DRUG INTERACTIONS
Cimetidine, benzodiazepines, calcium channel blockers, cyclosporine, remeron, nateglinide, nefazodone, sildenafil, tacrolimus, venlafaxine, lithium, phenytoin, phenobarbital, warfarin.

FOOD INTERACTIONS
  • Concurrent use of alcohol and metronidazole should be avoided as it can cause a disulfiram reaction (such as nausea, vomiting, flushing).
  • Flagyl ER should be taken on an empty stomach, at least one hour before or two hours after meals.

PREGNANCY AND BREAST-FEEDING CAUTIONS
FDA Pregnancy Risk Category B. May be contraindicated in first trimester. Excreted into breast milk. Unnecessary exposure to metronidazole in breast-feeding infants should be avoided. Consult your doctor if you are breast-feeding.

SPECIAL INFORMATION
This drug has a black box warning regarding a carcinogenic effect noted in animals. Avoid unnecessary use (oral and injection).
Vaginal formulation: Do not engage in sexual intercourse during therapy.
Topical formulation: Cleanse affected areas prior to application. Wait until medication dries before applying cosmetics to your face.

Flagyl has been shown to be as effective as oral neomycin, another antibiotic. Metronidazole therapy should be used for no more than two weeks to avoid peripheral neuropathy (a nervous system condition). It also can cause side effects such as dark brown or reddish urine, diarrhea, dry mouth, metallic taste in the mouth, headache, appetite loss, nausea, and mild stomach pain or cramps.

In a study conducted at the Bristol Royal Infirmary, Flagyl and neomycin were evaluated for one week in 11 patients with acute or chronic hepatic encephalopathy. Improvement in mental state, and reduction in tremors and bad breath were seen with both antibiotic drugs. Results suggest that metronidazole may be as effective as neomycin for hepatic encephalopathy. The use of Flagyl is recognized by the American College of Gastroenterology for patients with hepatic encephalopathy, but they recommend close, careful monitoring of the kidneys, nervous system, and the ears.
