DESCRIPTION
Olanzapine belongs to a group of drugs known as atypical antipsychotics. This drug is used to treat psychotic mental disorders such as schizophrenia, bipolar disorder, and agitation that occur with schizophrenia and bipolar mania. Side effects include sour stomach, belching, change in walking and balance, constipation, difficulty speaking, dizziness, drowsiness, dry mouth, headache, heartburn, runny nose, and sleepiness, among others.

CAUTIONS
  • May worsen narrow-angle glaucoma, or myasthenia gravis.

  • Notify your doctor if you plan to become pregnant, if you have heart disease; cerebrovascular disease or conditions that predispose to low blood pressure, history of seizures, thyroid disease, liver disease, increased cholesterol or triglycerides levels, diabetes.
  • Neuroleptic malignant syndrome (NMS), a potentially fatal complex, has been reported with antipsychotic drugs. Report any of the following symptoms to your doctor immediately: fever, muscle rigidity, altered mental status, irregular heart rate, increased heart rate.
  • Has been reported to cause significant increases in blood glucose levels, causing diabetes that requires treatment.
  • May disrupt regulation of body temperature. Avoid dehydration, particularly during strenuous exercise or extreme heat.
  • Patients with bipolar depression/schizophrenia may be at increased risk of suicidial thinking.
DRUG INTERACTIONS
Minimize risk
Carbamazepine: Decreased olanzapine concentrations.
Levodopa: Antagonism of the effects of levodopa due to dopamine receptor blockade.

FOOD INTERACTIONS
Avoid alcohol; it may increase sedation.

HERBAL INTERACTIONS
Dong quai, St. John’s wort, kava kava, gotu kola, valerian.

SPECIAL INFORMATION
  • This drug has a black box warning describing the risk of increased mortality when the drug has been used for off-label treatment of dementia in the elderly.
  • Use good oral hygiene; frequent rinsing of mouth, sugarless gum, candy, ice chips for dry mouth.
  • Avoid hazardous activities until drug response is determined.
  • Orthostatic hypotension occurs often and to rise from sitting or lying position gradually.
  • Avoid hot tubs, hot showers, tub baths, since hypotension may occur.
  • Avoid abrupt withdrawal of this drug, or EPS may result; drug should be withdrawn slowly.
  • Avoid OTC preparations (cough, hay fever, cold) unless approved by prescriber, since serious drug interactions may occur; avoid use with alcohol, CNS depressants; increased drowsiness may occur.
  • In hot weather, heat stroke may occur; take extra precautions to stay cool.
Olanzapine lacks the dangerous side effects of other antipsychotic drugs (i.e., clozapine); therefore, researchers have studied the effects of this drug in persons with Huntington's disease. Researchers in Israel used olanzapine for 11 patients (five of whom were men) with Huntington's disease. Nine patients were treated for 9.8 months. Disease symptoms improved in five patients; two patients dropped out because of drug eruption and lack of efficacy. It was concluded that this drug is a good alternative treatment in Huntington's disease, primarily for the psychiatric symptoms. Researchers in Spain described two patients with a predominantly "chorea-typed" Huntington's disease whose abnormal movements lessened after treatment with this medication. In these two patients, the drug was well tolerated.
