DESCRIPTIONOndansetron tablets are an anti-emetic, which prevents nausea and vomiting induced by anesthesia, chemotherapy, and radiation therapy. It may be administered by injection or taken in a tablet form. Side effects include diarrhea, headache, constipation, rash, lightheadedness, dry mouth, and drowsiness. Serious side effects include allergic reactions, muscle cramps or uncontrollable movements, and irregular heartbeats, all of which require immediate medical attention.
ORIGINAL USES (ON-LABEL)
To prevent nausea and vomiting associated with certain cancer chemotherapy and total body radiation therapy, prevention and treatment of post-operative nausea and vomiting.
NEWLY DISCOVERED USES (OFF-LABEL)
Alcoholism (cravings & dependence), tardive dyskinesia (drug-induced), multiple sclerosis, pruritus, schizophrenia, uremic pruritus.
POTENTIAL SIDE EFFECTS
Changes in blood pressure, anxiety, agitation, dizziness, drowsiness, sedation, headache, malaise, fatigue, chills, shivering, abdominal pain, constipation, diarrhea, dry mouth, cold sensation, numbness, itching, urinary retention, weakness, increase in liver function tests.
CAUTIONS
- Orally-disintegrating tablets contain phenylalanine.
- This drug does not increase gastric motility.
- May require reduced dosages in elderly patients.
This drug is metabolized by a specific set of liver enzymes (cytochrome P450 enzymes). Several other drugs interfere with these liver enzymes, and thus may increase or decrease the clearance of ondansetron from the body, potentially increasing the risk of side effects or decreasing effectiveness. When these drugs are given in combination with ondansetron, dosage adjustments may be needed. SPECIAL INFORMATION
In a New Zealand study in Journal of Pain and Symptom Management, two patients with debilitating, chronic nausea associated with MS were evaluated for symptom management. One patient, a 43-year-old woman with a clinical presentation of MS, began to experience persistent nausea without vomiting with intractable vertigo within a few months of disease onset. Her nausea worsened with positional changes, became constant, and resulted in anorexia, and a weight loss of 25%. Combination drugs including prochlorperazine, metoclopramide, cyclizine, haloperidol, and lorazepam were used to treat the nausea and vertigo but were ineffective. After Zantac was administered, however, the patient's nausea improved within one day. She had continued treatment for five years and gained total control of nausea and experienced fewer bouts of vertigo.
The second patient, a 35-year-old woman with MS, also experienced acute vertigo and nausea. Successful treatment with high dose oral ondansetron resulted in a 50% improvement in control of nausea. Based on this limited data, Zofran may control nausea and vertigo in patients with MS, thereby improving quality of life by minimizing discomfort.
