DESCRIPTION
Tamiflu is the first orally administered neuraminidase inhibitor to be approved by the FDA. The antiviral activity of oseltamivir against influenza A and B was supported by experimental challenge studies in volunteers who received intranasal inoculations of challenge strains of influenza virus. These subjects received oseltamivir or placebo shortly after viral inoculation. Compared with placebo, it significantly reduced the median viral titers 24 and 26 hours after initiating therapy and the median time to cessation of viral shedding. This antiviral effect was associated with significant reductions in symptoms and biochemical markers of host inflammatory response in the respiratory tract.
ORIGINAL USES (ON-LABEL)
Treatment of uncomplicated acute illness due to influenza infection in adults who have been symptomatic for no more than 2 days. Prophylaxis of influenza in patients 13 yr and older.
POTENTIAL SIDE EFFECTS
Nausea (without vomiting), diarrhea, vomiting, abdominal pain, dizziness, cough, headache, insomnia, fatigue.
CAUTIONS
- There is no evidence for efficacy of oseltamivir in any illness caused by agents other than type A and B influenza viruses. Data on treatment of influenza B infections are limited.
- The efficacy of this drug in patients who begin treatment more than 40 hours after the onset of symptoms has not been established. The safety and efficacy of repeated treatment courses have not been studied.
- The efficacy of Tamiflu in subjects with chronic cardiac disease, respiratory disease, or both has not been established.
In clinical studies in this patient population, no difference in the incidence of complications between patients receiving placebo and patients receiving this drug was observed.
No information is available regarding treatment in patients with medical conditions sufficiently severe or unstable to be considered at imminent risk of requiring hospitalization.
- Use of Tamiflu should not affect the evaluation of individuals for annual influenza vaccination in accordance with CDC guidelines (i.e., oseltamivir is not a substitute for vaccination). The efficacy of Tamiflu for prophylaxis of influenza virus infection has not been established.
- Oseltamivir and oseltamivir carboxylate are excreted in the milk of lactating animals. It is not known whether this occurs in humans. Therefore, the drug should only be used if the potential benefit for the lactating mother justifies the potential risk to the breast-fed infant.
DRUG INTERACTIONS
The low protein binding of oseltamivir and oseltamivir carboxylate suggests that the probability of drug interactions due to displacement from plasma proteins is low.
Interactions between Tamiflu and drugs that induce, inhibit, or are metabolized by cytochrome P-450 enzymes are unlikely because neither oseltamivir nor oseltamivir carboxylate is a substrate for or inhibitor of these enzymes.
Drug interactions involving competition for the esterases that metabolize it have not been reported extensively in the literature.
Studies of possible interactions between influenza vaccine and this drug have not been conducted. In studies of naturally acquired and experimental influenza, Tamiflu treatment did not impair the normal humoral antibody response to infection.

PREGNANCY AND BREAST-FEEDING CAUTIONS
Oseltamivir phosphate is classified in FDA pregnancy category C (animal reproduction studies have shown an adverse effect on the fetus, and there are no adequate and well-controlled studies in humans, but potential benefits may warrant use of the drug in pregnant women despite potential risks). Although animal reproduction studies may not be predictive of human response, oseltamivir phosphate should be used by humans during pregnancy only if the potential benefit justifies the potential risk to the fetus.
SPECIAL INFORMATION
- This medicine should not be used as a substitute for being vaccinated against flu, because the protection against flu lasts only as long as you take the medicine.
