DESCRIPTION
Promethazine blocks dopamine, alpha and histamine receptors to reduce nausea and vomiting, and produce sedation.

ORIGINAL USES (ON-LABEL)
To treat allergic conditions, vomiting, motion sickness, postoperative pain (in combination with other drugs), and anaphylaxis (in combination with other drugs), also used as a sedative and an anesthetic (in combination with other drugs).

NEWLY DISCOVERED USES (OFF-LABEL)
Hyperemesis gravidarum.

POTENTIAL SIDE EFFECTS
Drowsiness, sedation, blurred vision, confusion, dizziness, disorientation, extrapyrimadal symptoms, ringing in the ears, incoordination, fatigue, euphoria, nervousness, double vision, insomnia, tremors, changes in blood pressure or heart rate, dry mouth.

CAUTIONS
  • Not for use in coma, treatment of lower respiratory tract symptoms (including asthma) in children less than 2 years of age.
  • Avoid other drugs that cause respiratory depression.
  • When used in children older than 2 years, the lowest possible dose should be used.
  • Has rarely caused a potentially fatal syndrome called neuroleptic malignant syndrome, characterized by fever, muscle rigidity, altered mental status, irregular blood pressure, increased heart rate, sweating, irregular heart rate.
  • Notify your doctor if you have sulfonamide allergies, Reye's syndrome, heart disease, impaired liver function, asthma, sleep apnea, a history of seizures, or concurrently taking narcotics or local anesthetics. Phenergan can lower the seizure threshold.
  • Elderly are at an increased risk of side effects, particularly tremors, uncontrolled muscle movements.

FDA Pregnancy Risk Category C. Excretion in breast milk is unknown. Breast-feeding while taking this drug is not recommended.

SPECIAL INFORMATION
This drug has a black box warning describing that it should not be used in children less than 2 years of age because of the potential for fatal respiratory depression. In addition, when used in children older than two years of age, the lowest effective dose should be used. (See cautions.)

It has been compared with methylprednisolone for the treatment of morning sickness. Women with a normal-appearing pregnancy of less than or equal to 16 weeks' gestation with morning sickness received either oral methylprednisolone, or oral promethazine. After three days, methylprednisolone was tapered over the course of two weeks, whereas the promethazine was continued without change for two weeks. For women who continued to vomit after two days, the medication was discontinued. Women receiving either medication at discharge continued to take the remainder of the assigned medication from the packaged pill dispensers. They were followed up weekly.

There were no significant differences between the groups with respect to maternal age, gestational age at entry, number of previous admissions, or greater than five percent body weight loss. Three women in the methylprednisolone group and two in the promethazine group failed to stop vomiting within two days. One woman from the promethazine group was unavailable for follow-up. No women from the methylprednisolone group was readmittedbut five of the 17 women receiving this drug were readmitted for excessive nausea within two weeks of discharge. No adverse effects were noted for either drug and it was concluded that a short course of methylprednisolone is more effective than it for the treatment of morning sickness.
