DESCRIPTION
Quetiapine is used to treat psychotic symptoms such as hallucinations, delusions, and hostility. About 50% of persons with advanced Parkinson's disease exhibit psychotic symptoms which quetiapine can treat. POTENTIAL SIDE EFFECTS
Headache, sudden drop in blood pressure upon standing from a sitting or lying down position, somnolence, weight gain, increased heart rate, dizziness, rash, abdominal pain, constipation, dry mouth, upset stomach, back pain, weakness, sore throat, cough, shortness of breath, increases in triglycerides or cholesterol, increased prolactin levels, increases in liver function tests, priapism (prolonged painful erection).

CAUTIONS
  • Reduced doses may be needed if you have liver impairment.
  • Has demonstrated increased mortality in elderly patients when used for off-label treatment of dementia in elderly patients.
  • Not for use in central nervous system depression, bone marrow suppression, blood dyscrasias, severe liver disease, coma.
  • Notify your doctor if you plan to become pregnant, if you have heart disease, cerebrovascular disease or conditions that predispose to low blood pressure, history of seizures, thyroid disease, liver disease, increased cholesterol or triglycerides levels, diabetes.
  • Neuroleptic malignant syndrome (NMS), potentially fatal complex, has been reported with antipsychotic drugs. Report any of the following symptoms to your doctor immediately: fever, muscle rigidity, altered mental status, irregular heart rate, increased heart rate.
  • Has been reported to cause significant increases in blood glucose levels, causing diabetes that requires treatment.
  • Report any vision changes to your doctor, particularly blurred vision. May require regular eye exams to monitor for the development of cataracts.
  • Patients with schizophrenia may be at increased risk of suicidal thinking.
  • May disrupt regulation of body temperature. Avoid dehydration, particularly during strenuous exercise or extreme heat.

Minimize risk
  • Antihypertensives: Increased risk of hypotension.
  • CYP3A inhibitors (Azole antifungals, macrolide antibiotics): Increased plasma quetiapine concentrations.
  • Lorazepam: Increased plasma lorazepam concentrations.
  • Phenytoin, carbamazepine, barbiturates, rifampin, glucocorticoids (enzyme inducers): Decreased plasma quetiapine concentrations.
  • Thioridazine: Decreased plasma quetiapine concentrations.

  • FOOD INTERACTIONS
    Serum levels may be increased when taken with food. Avoid alcohol.

    HERBAL INTERACTIONS
    St. John's wort, valerian, kava kava, gotu kola.

    PREGNANCY AND BREAST-FEEDING CAUTIONS
    FDA Pregnancy Risk Category C. Excretion in breast milk is not known. Do not breast-feed during therapy.

    SPECIAL INFORMATION
    This drug has a black box warning describing the risk of increased mortality when the drug has been used for off-label treatment of dementia in the elderly.

    In clinical studies, quetiapine has shown promise as an effective drug for psychosis and improves psychosis in Parkinson's disease (PD) without triggering movement disorders. Researchers at Emory University School of Medicine in Atlanta, Georgia, evaluated the effects of quetiapine for psychosis in 29 patients with PD who failed to respond to treatment with clozapine, risperidone, or olanzapine. In this 24-week trial, researchers found significant improvements in psychosis and no decline in motor functions. Furthermore, there were significant improvements in recall scores on cognitive tests. Researchers concluded that small doses of quetiapine may treat psychotic symptoms and improve cognition without worsening motor function in patients with PD.
