DESCRIPTION
Avandia helps the insulin in your body work better so that the glucose from the food you eat can get into your cells where it is used for energy. Avandia and other diabetes medications are not cures for diabetes. They must be taken regularly to be effective. ORIGINAL USES (ON-LABEL)
Rosiglitazone is indicated as monotherapy as an adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus. It is also indicated for use in combination with metformin when (1) diet, exercise, and rosiglitazone or (2) diet, exercise, and metformin do not result in adequate glycemic control. For patients whose diabetes is inadequately controlled with a maximum dosage of metformin, rosiglitazone should be added to rather than substituted for metformin therapy.

Rosiglitazone is contraindicated in patients with known hypersensitivity to the product. This drug use in pediatric patients is not recommended because there are no data on use of the drug in patients less than 18 years of age. Further study in the female adolescent population is needed because thiazolidinediones appear to inhibit ovarian steroidogenesis.

Prevention of cardiovascular disease in type 2 diabetes, prevention in type 2 diabetes.

Weight gain, increases in total and LDL cholesterol, decreases in hemoglobin and hematocrit, edema

CAUTIONS
  • Liver enzymes should be measured before initiating therapy, and the drug should not be used in patients with clinical evidence of active liver disease or increased serum transaminase levels (ALT more than 2.5 times the upper limit of the normal range).
  • Liver enzyme monitoring is recommended every 2 months for the first 12 months of therapy and periodically thereafter.
  • Liver enzymes should be checked if the patient develops symptoms suggesting hepatic dysfunction (unexplained nausea, vomiting, abdominal pain, fatigue, anorexia, or dark urine). Rosiglitazone therapy should be discontinued if jaundice develops.
  • This drug should not be used in patients who experienced jaundice while taking troglitazone.
  • It is only effective in the presence of insulin. It should not be used in patients with type 1 diabetes or for the treatment of diabetic ketoacidosis.
  • It should be used with caution in patients with edema.
  • In premenopausal anovulatory women with insulin resistance, rosiglitazone therapy may result in resumption of ovulation. These women may be at risk for pregnancy if adequate contraception is not used.
DRUG INTERACTIONS
Some combinations—for example, Avandia and metformin (Glucophage) — are helpful. Others can cause problems.
Delavirdine, fluconazole, oral contraceptives, ketoconazole, nicardipine, NSAIDs, sulfonamides, itraconazole, carbamazepine, phenobarbital, gemfibrozil, phenytoin, rifampin, rifapentine, secobarbital, bile acid sequestrants.

FOOD INTERACTIONS
May take without regard to meals.

HERBAL INTERACTIONS
Hypoglycemia: chromium, coenzyme Q10, fenugreek.
Decreased antidiabetic effect: bee pollen, blue cohosh, broom, chromium, elecampane, eucalyptus, gotu kola.

PREGNANCY AND BREAST-FEEDING CAUTIONS
Rosiglitazone is classified in FDA pregnancy category C: Do not use unless the potential benefit justifies the potential risk to the fetus. It is not known whether rosiglitazone is excreted in human milk. It should not be administered to breast-feeding women because many drugs are excreted in human milk.

SPECIAL INFORMATION
  • Avandia can cause weight gain and puffiness in the lower legs and feet. Watch your diet and weigh yourself daily. Contact your health care provider if you gain weight unexpectedly or if you notice puffiness.
  • Avandia also can cause anemia—a decrease in the number of the oxygen-carrying red blood cells in your bloodstream. Contact your health care provider if you experience fatigue, lightheadedness, or breathlessness, especially during physical activity.
