DESCRIPTION
Tamsulosin is generally prescribed for men with benign enlarged prostate glands. It relaxes the muscle around the gland, freeing the flow of urine. In the treatment of stones, it may relax the muscles of the ureters, enabling the stones to pass.

POTENTIAL SIDE EFFECTS
Headache, dizziness, drowsiness, insomnia, decreased libido, runny nose, sore throat, increased cough, sinusitis, diarrhea, nausea, abnormal ejaculation, infection, back pain, chest pain, orthostatic hypotension (sudden drop in blood pressure).

CAUTIONS
  • Not intended for use as a blood pressure medication.
  • Marked orthostatic hypotension (sudden drop in blood pressure when standing from sitting or lying down position), syncope, and loss of consciousness may occur with first dose (especially in patients taking beta-blockers, diuretics, low sodium diets or taking large first doses).
  • Avoid rapid increase in dose.
  • Rarely, this drug class has been associated with priapism (painful erection).
  • Use with caution in patients with kidney or liver impairment.
  • Does not affect levels of PSA (prostate specific antigen), a marker test used to diagnose prostate cancer.

SPECIAL INFORMATION
In a study in Journal of Urology, patients received either supportive therapy (the control group) or treatment with nifedipine or tamsulosin. At the end of 4 weeks, 43% of the controls, 80% of those on nifedipine, and 85% of those on tamsulosin had passed the stones.

Another study published in Journal of Urology compared tamsulosin with floroglucine-trimetossibenzene (FTMB), a combination of antispasmodic drugs. In addition, all patients received a corticosteroid for anti-inflammatory benefits, an antibiotic, and a non-steroidal anti-inflammatory drug (NSAID) as needed for pain. After four weeks, all the tamsulosin patients had passed their stones, compared to 70% of the FTMB group.

In a trial published in International Urology and Nephrology, everyone received the local standard therapy, which included a painkiller, a tranquilizer, an NSAID, and an herbal; half also received tamsulosin. The stone expulsion rate was 80.4% in those on tamsulosin compared to 62.8% in those on standard therapy alone.

According to researchers, these studies suggest tamsulosin may have some benefit as an add-on therapy in treating ureteral stones; however, further study is needed. Physicians advise that it should be used with caution in those using other drugs that can lower blood pressure.
